What on earth is that?! I've had deformed sim glitches a few times now (deformed horses even more) but nothing like that. Is is even a sim?

It is a ghost toddler with a glitchy piece of CC. Something so simple can create such Lovecraftian monstrosities.

And kids, that's why you check your Sims3Packs when you download from the Exchange.
