Into the future travelling bug. In one of my save, I always ended up with no sims when arrive in the future.

IMAGINARY FRIENDS! I hate when they go in the bathroom and your sim is in the shower/toilet! Grrrrrrrr  >:(

I just hate the imaginary friends  - period.  There is no way to get rid of them.  No way to delete them.  I just stop getting the mail until my children become teenagers.  SOO annoying!!

I believe you can delete them with moveObjects on.

Also, completely random and stupid times for shows when you're playing a performer sim. Usually the further away the place you'll perform is, the later the show is...

I delete them with testingcheats also.  I think you shift+click on the IF and it displays the word 'object'.  You can delete it from that.

I hate it when I go all the way to a sim's place, get their relationship up high enough to take them to visit a lot, get there, and then they turn around and go home again straight after, before I even get to do anything with them!

Or when you go over to visit their place only to have them leave or work or whatever else before you can do anything. Or when you ask that sim out on a date, and they have to go to work five minutes later and you get a negative hit on the relationship for having a short date.

Related to Schip's post, when a townie throws a party and it's only them there. It's actually kind of sad. They wanted a party and no one returned their invites. :(

The two things that annoy me most are imaginary friends, and forgetting to put walls up before taking pictures. I get all set up, have the sims in the perfect spot, line everything up, then realize the walls are still down. >:(
All my sims seem to get imaginary friends, and I normally either just leave them in the sim's inventory, or delete them using testing cheats.

Sims autonomously playing with sprinklers in cold weather.
Sims autonomously talking to the maid, delaying her in what she's supposed to do.
Other sims calling my sims during NIGHT! What's wrong with them, seriously, does it sound normal calling someone at 22:30 for a chat?
Date calls. I seriously hate them, as anyone friend or better can annoy you this way. With this comes love letter and presents, forcing me to pretty often sell stuff accumulated in my family inventory. And yes, thes mostly happen when I'm sending my hard working sims to bed...
Party invites every second day when there are babies/toddlers in family. Especially the latest case with triplets. To make it better, many of the parties are scheduled for time my sim is working or short time after coming home. Actually 99% of parties are timed very badly.
I have 3 bookcases in the house. Why do sims autonomously go for the one furthest away from them, often even other floor?
Staircase width: 4. Sims using them at once: 3. And guess what? The line up, using the same path...
Cancelled wishes coming back often. No, I don't wanna buy a stroller or high chair, yet it comes up 3 times a day on each parent...

Rant over. For now, because this game will likely come up with something else...

How about bringing children to a large community lot after 22:00? Even if they're still on the same lot as their adult chaperon, they drop what they're doing and follow the adult if they happen to be on a certain distance away from them. Thankfully, it doesn't seem to happen with teens. They still need an adult on the lot so they're not considered to be breaking curfew, but having the adult play darts on one side of the lot and the teen play pool on the other is just fine. But my kids need some independence too!

Oh man, that drives me nuts too. And the kids can't even "Go Home" it's always "Follow _____." I wish we could cancel the action in favor of go home so I'm not wasting my time with this kid who is running all around the lot following someone.

Also, I get so annoyed by that Woohoo-off-lot strut. I sent someone to the theater to have a little fun, but it took them 4 hours to get home because they decided to walk half the way. Needless to say, she didn't make it home until 3 in the morning and she had to be at work by 6. If she had left when I told her to and went straight home without the walk she wouldn't be having issues with sleep.

Also, the "witnessed a breakup" moodlet is obnoxious. I had that one recently on an infant. A just-born-a-day-earlier infant. How can they even figure out what's going on? Toddlers I could understand that - infants? No.
