if we turn Bella Goth for example and then bump into a new sim called Barry von homeless we cannot change him unless we go right around the alphabet and back to B again right?

Correct. Ba comes before Be, so Barry would need to be turned first. Since you turned Bella, you'd then be looking for a Blake, Brenda, and maybe even Bjorn Ironside. Lol. "You vampires are nothing! I am the son of Ragnar Lothbrok!"

Except you don't start over when you get to the end of the alphabet. It's only one time through.

next year, do it reverse alphabet ie start with z and go to a (and solely by surname)

You can turn any Sim that your vampire meets, as long as you follow the alphabet.

This was fixed in a patch so he should always be called Johnny Zest now. If he's ever referred to as Zest Johnny it's a bug, which would make it odd to rule for.

It was not stupid at all, Rachel. In fact, truth be told, when Lena originally threw the idea out to the team I had it in my head that alphebetizing any list of names automatically goes to the surname. But, once I wrapped my head around the first name approach -- it's honestly simpler.
