If I remember correctly, I use Nraas MasterController for this.

It's been awhile since I've used it, so trying to give instructions might be a bad idea at least at the moment.

You can set family relationships with the Master Controller mod.  Since it's been a while since I've done it, I can't remember if you'll need to create a placeholder sim as the parent of the grandchild, though. I don't think you can directly have it say grandparent/grandchild without a sim in the middle to be the parent/child of the other two sims.  You could just create that extra sim in CAS and then use master controller to kill them so that they're just a placeholder with a tombstone that won't break the family tree.

What you described should work, although you will probably need some extra steps.  Here's how I'd attempt it:

Create the sims in CAS. You will have to make the grandparent an elder, the parent an adult or YA, and the child no older than teen. Then set their relationships accordingly. Unfortunately, you can't set adults as parents of YAs in CAS, I'm pretty sure.

Play this family in a throwaway save.  Start the game and turn on testing cheats. Shift-click on the teen to trigger age transition and make them YA (assuming this is where you want to start.) Shift-click on the parent sim to trigger age transition until they're elder, and then do it again at elder.  This should kill the sim of old age and generate a tombstone you can put in one of the sims' inventories. Or if you wanted them to remain adult or YA, you could just pick another death like starvation, by dragging the hunger bar to zero and waiting it out, or having them work out until fatigued and go swimming, and let drowning do them in. Or set a fire, etc.  Re-dress the YA grandchild how you want at the dresser/mirror (or edit in CAS cheat), and if the age-up didn't give you the traits you want, use testing cheats to change the traits.

Once the family dynamic is correct, make sure the dead parent's tombstone is in either the grandparent or child family inventory.  Go into edit town and save the household to the bin.

Now you should be able to place your family from the bin when you start a new game.

what i was trying to do is create my own real life story. my mom died at an early age and i was raised by my grandmother. i never went to college in real life, but wanted to try it in this game. being raised my my grandmother (elder) and as a ya i would go to college straight from high school. do you think i would be able to pull this off? what i was going to do is kill the mom(my mother died when i was 4. so i wanted to play out my real life here) and place her at the cemetary and then carry on the grandmother and teen from there.

You should be able to do that. Once your family is in the world you want to play in and the mother sim is dead, you can take the gravestone out of the inventory and place it in the cemetery. Make sure you put it on the ground on that lot if you want the ghost to spawn or be able to mourn at the tombstone. If it's in the mausoleum, that won't be available.

I did this with a sim - had him grow up with his grandmother. You do indeed need a "middle sim" to get it right, and I created a whole, realistic family now that I was at it, and then killed them off and placed the tombstones in the cemetary.

Just to make sure that grandma didn't die of old age before I was ready, I tweaked the age settings. Later on, I learned how to make Ambrosia. That way, I had her for a loooong time! She knitted sweaters for several generations before she went...

My sim became an immortal vampire, btw, and he maxed most skills and is the patriarch of a family that counts 11 generations. Yep, the unlucky, neurotic loser with the big ears and the freckles that got mercilessly bullied by his evil and hot-headed big brother got far! :D I think it's awesome to make backstories like that, and the initial making of a whole, realistic family and a few photos in the family album before they went made it even better for me.
