So it's my first sim in sims 4 and he got engaged. after 3 or 4 days they had their wedding, ate the cake, vowed and got a gold medal BUT sometimes both my sim and his wife want to get married to each other??? and when i look at their relationship it says "Spouse". they also have different surnames as well as their daughter. what should i do to fix it? or is it normal?

surnames are not that big of a thing but i dont want them to be spouses i want them to be wife and husband you know?

They are husband and wife. That's what spouse means.

It's been a glitch for as long as I've been playing that even after getting married spouses will still have whims to get married or engaged or become boyfriend/girlfriend. The only solution I know of for those glitchy whims is to cancel them.

To change their last names open up the cheat panel with control alt delete...

Then type "testingcheats on" enter

Then "cas.fulleditmode" enter

Then shift click the sim you want to change

Once in CAS, you should be able to change their last names.

@MudCan --- Welcome to the forum! If you let your married Sims renew their vows, it makes them happy, which is nice.

By the way, you're not dumb.  Asking questions is O.K. .

These whims are easily cancelled. My married sims get them regularly. Just cancel the whim and a new one will pop up. I suppose because it is not really a game-breaking bug it is low on the list of priorities of bug-fixes.

Sims no longer take their spouse's last time when they marry but I think that may have been intentional on the devs part. 

Even married Sims still pop whims to marry their husband/wife or to get engaged to them, it's kind of annoying, my marrried couples get this all the time, I just simply click them away.
