I haven't had a Sim give birth at the hospital, but from my experience with the Baby Boomer challenge I've found that the pregnant woman is always massive, regardless of whether she's going to give birth to a single baby or twins, and that her size doesn't change depending on which it is.
If you play the scientist career all the way through, I've read it will give involve aliens and alien objects at some point, so you'll get to do both at once.
Thank you for the input! I don't know why I thought her size would mean more babies in The Sims 4. It's never been confirmed. Maybe I played for too long and my brain went weird.
I haven't looked into any other career yet, so that's good to know! Thank you. Scientist is the one I want to do next, since I am least excited about the medical career.
From the sound of it, Plum meant when a Sim goes to the hospital that was added with the Get to Work expansion to give birth. Apparently that's an option now.

The baby was placed into a hospital crib, and then she disappeared and my Sims traveled home where the baby was waiting in a bassinet. It would have been nice for her to carry the baby out of the hospital, but I still liked all the interactions.
