I tested this out with my new legacy family who happened to be preggers and yes, home birth does give a huge boost to the friendship level of the mother and infant.

I hardly ever take my sim to the hospital.  They do tend to slip past you sometimes, so I just let them go.

Yeah I had to cancel the action of "Go to Hospital" and then the husband went to sleep. He stopped freaking out for some reason and tried to take a nap.

I had a Sim go into labor while jogging.  The message says to get her home or to the hospital to have a safe birth, so I just left her there on the side of the road to see what happened.  She had the baby and then they both just *poofed* back home.  No big deal.  I was a little disappointed.

ROFL, what exactly were you expecting Pam?  That's too funny.  Drop a kid and poof.  You made me laugh.

I always have mine at home.  Dad gets out of the way fast, mom just walks into the nursery, does what she must, and tucks the baby into bed after a feed and diaper and every one is happy.

Yes, it was similar to a reset poof.  One minute she was standing on the sidewalk spinning to have a baby, then the next instant she was in the back yard of the house and the baby was on the ground.

Lol, Joria!  Well, you get that dire warning that the mother needs to be at home or in the hospital to make sure she has a safe birth.  I was hoping there would be some kind of drama or something exciting or funny.

Yeah, my sim today had her baby at home, but I couldn't watch. I'm such a wimp.  :-[ It looked and sounded so painful. There was a slight boost to relationship, but I was so peeved I'd had a girl I grew her up with cheats and kicked her out. I'm horrible.  :-\
