Is that a Chance card like from Monopoly?

It's when the game throws up a scenario and you've got to pick one of two options. It does it when you are exploring the catacombs, for example, and sometimes you get them for work.

What expansion did it come with? Are you referring to those chance cards that happened with Sims 2? I haven't played in a long time, because I'm still waiting to get Supernatural.

I'm sure they've been there since the very beginning... There's not much about them, but they are on the wiki down the bottom of the page

This is the closest picture that I can find to what I'm talking about:

.

If you go and explore the catacombs in the mausoleum, you'll see what I'm talking about.

Now I feel like a complete idiot! I thought they were something different, like from Sims 2.  :-[

Never had that happen to me yet, but then I seldom enter rabbitholes.

What happens after you've chosen for Samuel Goth? Did the game report his adventures to you?
"Woohoo! British Nina!"
"Eep! He bid sold toy!"

Nope, sadly nothing else happened. He just left a short while later, with no further popups or notifications.

A similar glitch that pops up at times:

Occasionally I get the “Three’s A Crowd” message like when you have two Sims flirting with others in the same group. I often get them when my single Sim’s doing solo stuff at home. I have a feeling it involves Twallan’s StoryProgression mod which I use but I haven’t found a seemable option to disable it.

The chance cards, no, unless my Sim was doing the action. I almost always play with a single Sim so maybe another Sim in the household is triggering it?
“When life gives you lemons, don’t make lemonade. Make life take the lemons back!” - Cave Johnson, Portal 2
“You ought to upgrade to Windows 7!”, they said. “It’ll help your games run so much better!”, they said. If I see them again, then they had better know how to stabilize an on-the-fritz copy of The Sims 3 that was working just fine on Windows XP.

I got that particular glitch last night in my Life States Dynasty. Both parents and all three girls were in bed asleep, and as soon as a vampire gnome showed up, I got the message that three's a crowd, did I want to disband?

I just hit yes and went on my way.

Who first started calling them Chance Cards?  Is that an EA term?

Not sure who coined the term, but it is on The Sims Wikia page.

It's an EA term - in TS2, they were referred to by EA as Chance Cards regularly. They resembled these, but only appeared in careers. If you've played the Political Career, an example is the popup to steal campaign funds. In TS2, a Sim had two options (eg "Steal" or "Don't Steal") plus an option to ignore. Ignoring did nothing. When a Sim picked an option, there was a 50/50 chance (roughly) to get a good or bad result. A certain reward increased it to 80/20. So, for instance, this could happen:

Sim chooses to Steal. Good result. Sim gets $5000.
Sim chooses to Steal. Bad result. Sim loses job.
Sim chooses not to Steal. Good result. Sim gets 2 Logic Points.
Sim chooses not to Steal. Bad result. Sim's relationship with a random Sim goes down.
