I was going along fine with thequest until the Physician had to perform surgery.  There doesn't seem to be any way to do that!  Help, anyone?

Have you clicked on the Operating Table in your Physicians Clinic? It's a big wooden machine. If you have deleted it you can buy a new one in the Professions section of Domestic Area of Buy Mode.

Yes I have.  I finally had to abandon the quest.  Is it possible that the Physician has to reach a certain skill level before he can perform surgery?

I don't think they need any certain skill level, but a patient has to be available who has been injured and is not just sick.  If there aren't any sims like that in the clinic then the physician can't perform the surgery.   But if there were and the physician still couldn't do the surgery, it could have been a bug.  I've had quests where my knight lost all 'knight' interactions and I had to quit and restart for him to get them back.

I know I am rather late with this answer, but in order to do surgery your Physician needs to have 'Anesthesia' stocked in their inventory or stocked in the operating table.  Anesthesia is Lordleaf, Sagewort, Valoroot and Englewood.
First you have to stabilize the pain via the 'Apply Anesthesia' interaction by moving the marker into the golden sweet-spot area (pop-up in the upper left hand corner), then you can operate.
My level 2 Physician has operated successfully on several of my heroes when they went to her and asked for surgery after being gored by a unicorn or boar and after being mauled by a bear :o.  She has also performed surgery on patients in the clinic at this level.  Sometimes a hero may have to be operated on twice to be fully healed ;).

I am so glad I found this thread, it's a quest I haven't tried yet, but I plan to.
