 How do you know when the stuff has download from the Exchange? I click on the little 'Download' button, and it comes up with a window, I click 'Launch Application' and the The Sims 3 Game Launcher comes up. I cannot find anything from there! Is it just in game? 

Also, I've created some patterns. How do I get them onto the Exchange and into my game?

Welcome to our Forum, eastbankbabs.

When you download an item from the Exchange, the Sims 3 launcher will open.  Click on the Downloads button on the left side of the launcher and it will show you the items you've downloaded.  Sometimes you have to click the box next to each item and click Install, but not always.  Often they will install automatically after download.

As for the patterns, another member will have to help you with that because I haven't used Create A Pattern.

It didn't work for me. But I did find out that I have to 'Save File'. I haven't actually tried it in game but it says it's installed :D
But, how to UPLOAD?  ???

Directly under "downloads" there is a tab called "Uploads" Go to it, and click the box next to what you want to upload. You'll need to give it a title and description.

It may be different in the Create A Pattern program, but from within the game, you have to "Share" your items (Sims, lots, furniture, etc.) to make them available to the Exchange.  This is done from Create A Sim or Build/Buy mode.  Then from the Sims 3 Launcher, click the Upload button, then check the boxes of the items you want to upload, then click Upload.  We also have our own Swap Shop where you can share items you've made.  It's a different process to upload, but not really any more complicated.

Have you read the Create A Pattern Guide?  It may have some information that will help you.
