Well, I know now that the hero thing they are talking is, when my sim got her key to the city, they named her a neighborhood hero, she got a +50 moodlet for 5 days from that.  Her meter is always full.

Key to the city? Is that something you can unlock?

It's a symbolic "key to the city" kinda ceremony important people can get.
I've only seen it on TV and never in any European show for that matter.

I've gotten the Laundry Gnome twice now.  He doesn't do anything more special than regular gnomes, except that he's a cute little teddy bear.
