I checked on the card through Google and according to
NotebookCheck.net it looks like it's an older laptop graphics card, first released in 2007. Given that the card is about 7 years old, it could just be failing from normal use. Unfortunately, given that it is a laptop graphics card, if you are running it on a laptop there's no way to replace it. If you're running on a desktop, you can replace the card. Let us know what type of system you're running and we can go from there.
