How do I build the buildings I need to build?

You have to build the clinic which is with the wizard's tower.

I believe the clinic is separate from the Wizard's Tower when purchasing, then they come together when both have been placed.

Btw, while you can always "see the physician" while playing a monarch and getting him gored with unicorns, the odds of successful surgery are MUCH higher when you've spent some time playing and leveling the physician.  So first thing, I did some quests with the physician; makes playing the monarch, knight, and spy much easier when they don't carry -40 injury buffs for days.
