Now I know some games for pc you have to pay for the online features. For sims 3 do we get access to the exchange free or do we have to pay some sort of monthly subscription of some sort?

On the official Sims 3 site, there is a free Exchange and also a Store where you can purchase safe authentic Sims 3 items.  The Exchange is risky because there is often third party custom content there that can wreck your game.  We always advise extreme caution when downloading anything from the Exchange.  The Store often has free items, but it's generally based on SimPoints, which you purchase with real money.

Ah so basically the sims points are used for "dlc" more or less. Gotcha.

Is there a decent majority of items that you don't have to get with real money? I have sims 3 for ps3 and there's quite a bit of items but I would expect more in the pc from the start..

You will also get a 1000 sim points to spend at the store if you register the base game, same with World Adventures. However they stopped doing it after that. Wait, did they do it for HEL? Can't remember now.
