Disappearing Sim

I have a family of 6. On the console her picture became blank, but I could still control her and see her. I saved and quit and when I started again, she had disappeared. At first I thought, well her family have been spared the heartbreak, but she's still listed on everyones 'friend' list.
Since I'm playing the Immortal Dynasty I'm out of luck if she continues to count as living on the lot. Can anyone tell me for certain? Should I restart now?

I did try restarting from a backup, and thought I had saved myself, but then the same exact thing happened :-(

Any other thoughts or hints?

I'm hoping it is a patch issue. I had trouble with patch 4.5.6.009001 so was maybe overcautious in patching once I uninstalled and reinstalled.
I'm up to the newest patch now and starting again (tried going back to existing family and River had become invisible). Whats weird is, I don't have LN, but now all my sims want a hot tub!

Is her portrait (even if blank) still there? what happens if you resetsim her?
