I had a similar problem and noticed that it often happened at a moment when a sim needs to change an outfit - Trip had good advice that helped.  Pause the game and let the outfit load and when you unpause they should complete the action.  This is the thread http://www.carls-sims-3-guide.com/forum/index.php?topic=14625.0.  Saved me a lot of time, let me tell you!   :)

Sorry, for not replying any earlier, it just slipped out of my mind. I did manage to try that suggestion out, and it did work. I did find out the you didn't need to pause for 30 real life seconds, 5 to 10 is enough.
