Confusing problem styling

I'm starting the stylist career for the first time and my second client was Carly Benton in Lunar Lakes. She wanted a new everyday look. When I accepted the finished look and it came back out into the world, she was wearing something completely different from either her original outfit or what I'd picked and it said that I hadn't done what she'd asked. My first one had been for two new everyday looks and I did that fine (though it said the client didn't like it), the only difference in what I did was that this time I changed her hair slightly as well. I can see how that might cause it to fail me because while I gave her a new outfit I did something else as well, but again the hair style she came out with is a third one. I saw a picture in the Stylist guide of a badly dressed clown - is it that if you fail badly enough, they come out in a game-picked costume of awfulness?

That's what it sounds like. You're more likely to get bad makeovers when first starting out, but I think I got a bad one around level 5 once.
