Random drowning?

I had one of my sims just randomly drown at the poolside club in UC. There was clearly a way for him to get out, and his needs were maxed. I send him there just a short time before in the motive mobile. Has anyone ever had this happen? He wasn't hydrophobic or anything. How could this have happened!?
