Yesterday I was playing with the Islo Paradiso Altos. The biography says that Alphonso Alto cheated on his wife Lopita with Florita, but Lopita actually has no relation to the family at all. Alphonso's kid has no mom on the family tree.

Is that a developers mistake?

Yes. A few families in Isla Paradiso have that quirk. I think the Hans and the Scotts have the same issue with one parent not connected to their kid(s).
